Candidate Qualifying Forms

The Town Commission Candidates must file the two forms listed below before officially qualifying:

This form must be filed with the qualifying officer before the acceptance of any contribution or the expenditure of any funds by the candidate, including expenses by the candidate from his or her own personal funds.

APPPOINTMENT OF CAMPAIGN TREASURER DS-DE 09

This form must be filed with the qualifying officer within 10 days of having filed the DS/DE 9 form.

STATEMENT OF CANDIDATE

Campaign Treasurer Handbook & Reports 

Once a candidate has filed the DE/DE 9 form, the qualifying officer will notify the candidate and his or her treasurer of the reporting dates for Campaign Treasurer's Reports.

These reports show who contributed money to the candidate and how the candidate spent the money. They, too, are filed with the qualifying officer.

The qualifying officer can guide the candidate on which forms to file.
